COVID Patient Instructions to Prepare for Appointment

Read these instructions before your appointment

  • Review symptoms before leaving home- Feverish, coughing, shortness of breath, flu-like symptoms, muscle fatigue, headache, sore throat, loss of taste or smell, rash?
  • Please do not arrive early to your appointment. You will be asked to remain in your vehicle until your appointment time.
  • Please arrive to your appointment 5-10 minutes early.
  • Bring a mask with you that will cover your nose and mouth. If you don’t have one Rejuvent will provide you with a level 2 mask.
  • Arrive to Rejuvent alone unless pre-arranged 24 hours prior to appointment.
  • Call 480-889-8880 and let us know of your arrival and remain in your vehicle.
  • Front office staff will begin the check-in procedure over the phone and complete a wellness screening before you can officially be checked in for your appointment.
  • During your wait, your assigned coordinator will give you a call to review your specific needs for the appointment. If you leave the office please allow for a quiet, undisturbed conversation to help your appointment go more smoothly.
  • When we are ready for your appointment to begin a Rejuvent staff member will call you and invite you to the front door and wait for you to put on your mask.
  • The mask must remain on for the duration that you are in the building, unless instructed otherwise. Refusal will be cause for cancellation of your appointment immediately.
  • Please only bring in a wallet or small purse and your cell phone only.
  • You will be offered a bottle of water when you come into the building, please leave personal water bottles inside your vehicle. No coffee service at this time.
  • The assistant or provider will give you some hand sanitizer and take your temperature.
  • Once the assistant is finished with your assessment, she will invite you to the front counter to sign your consents.
  • You will be escorted to the photo room. Please wash your hands while in the photo room and wait for the coordinator to arrive.
  • If you need to leave the photo room or procedure room please notify the front desk by calling 480-889-8880 and we will have a staff member go with you. Wandering around is not permitted.
  • The check out process will happen in the treatment room with the assistant or coordinator.
  • The assistant will bring all products that are purchased to the procedure room and they will walk you out.
  • Tester bars have been discontinued temporarily.
  • A New Beauty magazine will be offered to you as we walk out only.
  • Expect a phone call to reschedule your next appointment.
